The evidence of disability documentation should include:
- The name(s), title(s), professional credentials, address(es) and phone number(s) of the assessors as well as the date of the report
- A clear statement of the disability or significant ongoing illness, with supporting data
- A summary of assessment procedures and evaluation instruments used to make the diagnosis, including evaluation results and, if applicable, standardised scores
- A summary of present symptoms which meet the criteria for diagnosis of the disability
- Information relating to the student's needs and the status of the disability or significant ongoing illness (static or changing) and its impact on the demands of the academic programme
- Suggestions of reasonable accommodation(s) which might be appropriate in the context of further or higher education. These recommendations should be supported by the diagnosis
