This downloadable form must be printed down in full, completed, signed and returned by post to your local authority or local VEC, as appropriate, in the area in which you live.

The following instructions apply:

1. Download and read the accompanying guidance notes before completing the application form.
2. Print down the application form and complete it in full, or alternatively download the application form to your computer, complete on-screen and print down. Please remember to safeguard your personal details. Should you choose to complete the form on-screen on a public computer, please ensure that you delete the file and clear temporary internet files and your browser history before leaving the computer.
3. This form must be printed down in full.
4. Ensure the declarations are signed. The application form is not valid and cannot be processed without these signatures.
5. Attach the supporting documentary evidence securely to the completed application form.
6. Attach a course acceptance form (FA1, FA2 or SA1 form) to the completed application form.
   
7. Return by post to your local authority or local VEC, as appropriate, in the area in which you live.
